  • Infections like Meningitis, Lyme Disease & Tuberculosi
  • Toxic Optic Neuropathy caused by alcohol or tobacco

SYMPTOMS OF NEURO-OPHTHALMIC DISORDERS

  • Sudden or gradual vision loss in one or both eyes
  • Blurred or dim vision even with glasses
  • Eye Movement & Coordination Issues
  • Drooping eyelids (Ptosis) in one or both eyes
  • Unequal pupil sizes (Anisocoria)
  • Abnormal pupil reactions to light
  • Dizziness & balance issues

Common Neuro-Ophthalmic Conditions

Neuro-Ophthalmic Problems We Treat

Optic Neuritis

Inflammation of the optic nerve, caused by multiple sclerosis or infections. It may lead to sudden vision loss, pain, and changes in color vision.

Treatment:
  • Steroid Medications - used to reduce inflammation in the optic nerve and limit damage
  • Intravenous (IV) Steroid - used to quicken the recovery
  • Plasma Exchange (PLEX) Therapy - used to reduce inflammation and recover vision
Optic Neuritis Inflammation

Papilledema

Swelling of the optic nerve due to increased intracranial pressure. It is caused by brain tumors, head injuries, or meningitis.

Treatment:
  • Acetazolamide - a medication used to reduce fluid buildup
  • Corticosteroids - a steroid used to reduce inflammation
  • Optic Nerve Sheath Fenestration (ONSF) - a surgery that creates an opening in the sheath surrounding the optic nerve to relieve pressure
  • Cerebrospinal Fluid (CSF) Diversion - a surgical treatment where a shunt (a long, thin tube) is placed to drain excess Cerebrospinal fluid
Papilledema Problem

Compressive Optic Neuropathy

It occurs when a tumor or other mass presses on the optic nerve. It can cause gradual vision loss.

Treatment:
  • Corticosteroids - medications used to reduce inflammation caused by compressive optic neuropathy
  • Orbital Decompression - a surgery that relieves pressure on the optic nerve by removing the mass that is compressing it
  • Radiation Therapy - used to shrink tumors that causing compression as well as for tumors that are difficult to reach surgically
Compressive Optic Neuropathy

Myasthenia Gravis (MG)

Also known as Neurogenic Double Vision, it is an autoimmune disorder that weakens the eye muscle and leads to double vision and droopy eyelids.

Treatment:
  • Corticosteroids - medications used to suppress the immune system and reduce the production of antibodies
  • Immunosuppressants - other medications that reduce immune system activity
  • Thymectomy - surgical removal of the thymus gland to rebalance the immune system
  • Plasmapheresis - a procedure used to remove the harmful antibodies from the blood plasma & replaces them with donor plasma
  • Intravenous Immunoglobulin (IVIg) - is given intravenously to stop the immune system from attacking the nervous system
Myasthenia Gravis (MG)

Diabetic Neuropathy

It is a type of nerve damage that may occur in diabetic people. It can damage the eyes leading to conditions like diabetic retinopathy as well as other eye problems.

Treatment:
  • Corticosteroids - medications injected in the eye to reduce inflammation and swelling
  • Photocoagulation - a laser surgery used to seal the leaking blood vessels or stop the formation of new blood vessels
  • Anti-VEGF Injections - injected in the eye to slow the growth of abnormal blood vessels & reduce fluid buildup
  • Vitrectomy Surgery - a surgical procedure that removes the vitreous (a gel-like substance) inside of the eye, and replaces it with a clear liquid
Healthy Retina vs. Diabetic Neuropathy

Nystagmus

It is a neurological condition in which the eyes make rapid, repetitive, and uncontrolled movements. It is caused by inner ear problems, brain diseases (like multiple sclerosis), eye or brain abnormalities and drug overdose.

Treatment:
  • Prism Lenses - special lenses that limit the amount of eye movement to see clearly.
  • Vision Therapy - Exercises to improve visual tracking & coordination
  • Medications - Gabapentin for anti-seizure, Baclofen as muscle relaxant & Onabotulinumtoxina to temporarily paralyze overactive eye muscles
  • Eye Muscle Surgery - improves eye movement and reduces the need to tilt the head to see clearly
  • Laser Vision Correction Surgery - If a person has nystagmus and is nearsighted, laser vision correction can reduce nystagmus symptoms

Frequently Asked Questions

What are neuro-ophthalmic diseases, and how do they affect vision?

Answer: Neuro-ophthalmic diseases are disorders that affect the connection between the brain and the eyes. It includes conditions like optic neuritis, papilledema, and strokes and can lead to vision loss, double vision, or difficulty in controlling eye movements. Early diagnosis of neuro ophthalmic disorder is essential to prevent permanent damage.
Answer: Treatment of neuro-eye conditions depends on the cause. Some conditions, like optic neuritis, may improve with steroids, while others, like optic nerve damage from a stroke, may be irreversible. Early diagnosis, lifestyle changes, and therapy can help manage these conditions.
Answer: Yes, they are often associated with neurological disorders like multiple sclerosis (MS), stroke, brain tumors, or diabetes. A comprehensive evaluation is necessary to determine the underlying cause of such conditions.
Answer: If you have unexplained vision loss, double vision, abnormal pupil responses, or symptoms like balance issues or numbness, a neuro-ophthalmologist can provide specialized care and treatment for it.
Answer: Yes, some conditions, like optic nerve damage due to glaucoma, stroke, or severe optic neuritis, can lead to permanent vision loss if not treated early. However, timely intervention and proper management can help preserve the vision and prevent further damage.
Answer: Maintaining a healthy lifestyle can help prevent and manage neuro-ophthalmic conditions. Some tips include controlling diabetes and blood pressure, eating a balanced diet rich in antioxidants, avoiding smoking and alcohol, managing stress, and getting regular eye and neurological check-ups.
